Ordering the PATCH CORD

Currently we offer a number of ways for you to order the PATCH CORD.  You're welcome to place your order via telephone.

• Simply call us at one of the above phone numbers 9AM - 5PM (Eastern Time or GMT -05:00) Monday - Friday, and we'll be glad to take your order over the telephone.
• Fill out the online order form, print it out, and fax it to us.
• Fill out the online order form, print it out, and mail it to us.

All orders received by us prior to 2PM (our time) will be shipped the same business day.

About the Order Form

To complete the order form, simply fill in all the fields (email address is optional).  When done instruct your web browser to print the whole page ("CTRL+P", the "print" button, or go to "file" | "print"), then be sure to sign the document prior to faxing or mailing it to us.  If you have any problems or questions please feel free to contact us via telephone or email.
